How do you deal with teachers who never give 4.0's to anyone on any assignment? ...meaning, how do I get a 4.0?

Ask them what they want for you to get a 4.0. Get their breakdown of the grade. If they don't have one, and are just arbitrarily giving out grades, then give them a bad review (if your institution has those).

If by 4.0 you mean A, then yeah, find out their requirements and follow them exactly. If you do and they still don't give you an A, sue.
